* A deposit of $50 is due at the time you schedule a new patient consultation. This deposit will be applied to the cost of the consultation on the day of your visit. We require 48 hours' notice for cancellation of a new patient consultation. In the event that a new patient consultation is cancelled less than 48 hours before the appointment or you are a no show, this deposit will service as the late cancellation/no show fee.

These prices apply to direct care patients only. Medicare beneficiaries will have their insurance billed first. They will be responsible for any balances that Medicare determines are their responsibility.
We are now out-of-network with all commercial insurance policies. We are only participating in traditional Medicare and most Medicare supplement plans. If you have a commercial insurance or Medicare Advantage policy, you will be subject to our direct care fees, which are due at the time of service. You may be eligible for reimbursement from your insurance plan if your have out-of-network benefits. You should contact your insurance plan directly about what your out-of-network benefits include.
