Here you will find some of our new patient intake forms. Feel free to download and print a copy to bring with you to your visit. We also have a patient portal where you can see your health records and complete your online check-in for your visits. The website for the portal is https://23151.portal.athenahealth.com/ .

Please note: The Medical Records Release Form is only necessary if we need to obtain records from other physicians for your visit. If you have seen another rheumatologist in the past, we will need those records prior to your appointment.
