Here you will find some of our new patient intake forms. Feel free to download and print a copy to bring with you to your visit. We are transitioning to a new electronic medical records at this time. Please call the office if you have any questions about this process. 

Please note: The Medical Records Release Form is only necessary if we need to obtain records from other physicians for your visit. If you have seen another rheumatologist in the past, we will need those records prior to your appointment.
